Patient Resources
We help you understand your dental benefits so you can focus on your smile, not the paperwork.
Figuring out what your dental plan actually covers can feel like reading a foreign language, and we don't think you should have to decode it alone. At Havens Family Dental, our front office team handles the paperwork side of your care so you can focus on your smile instead of spreadsheets and claim codes.
We currently work directly with Delta Dental and Anthem Blue Cross Blue Shield, and we also welcome patients with PPO insurance as an out-of-network provider. Whatever plan you carry, our team files the claim on your behalf and keeps you informed every step of the way, from your first cleaning to a planned restorative procedure.
If you have questions about your specific plan before your visit, we're happy to look into it for you. A quick call to our Blacklick office with your insurance information lets us give you a clearer picture of what to expect financially before you ever sit in the chair.
We built our insurance process around one goal: removing the guesswork so you can get the care you need without financial surprises.
Our team submits insurance claims directly to your carrier after every visit, so you aren't stuck filling out forms or waiting on hold with your insurance company.
We're in-network with Delta Dental and Anthem Blue Cross Blue Shield, and we also see PPO patients as an out-of-network provider, often with similar reimbursement.
Before any recommended treatment, we walk you through what your plan is likely to cover and what your out-of-pocket responsibility will be, with no hidden line items.
Coverage, deductibles, and annual maximums vary quite a bit from plan to plan, even among patients who carry the same insurance company. The list below reflects the providers we currently work with most often, but we encourage you to call our Blacklick office and give us your plan details so we can confirm your specific benefits before your appointment.
Don't see your plan listed? That doesn't necessarily mean we can't see you. Many PPO plans outside our direct network still provide meaningful reimbursement, and our team can give you a good-faith estimate of your coverage before you commit to treatment. Call us at 614-762-2737 with your insurance card handy, and we'll do the legwork.
Most dental plans share a few common building blocks: an annual maximum (the total amount your plan will pay in a calendar year), a deductible (the amount you pay before coverage kicks in), and a set of percentages for preventive, basic, and major services. Preventive care, like your routine cleanings and exams, is typically covered at or close to 100 percent, while procedures like crowns or root canals are usually covered at a lower percentage.
Because every employer and plan negotiates slightly different terms, the same insurance company can cover two patients very differently. That's why we never assume. We look up your actual benefits whenever possible and explain them in plain language, not insurance jargon, so you know what to expect on your statement.
Here's what our team handles for you at every visit:
If you're a new patient weighing your options, our New Patients page walks through what to bring to your first visit, and our Financial Info page covers payment plans for anyone without insurance or with coverage gaps.
"Our patients shouldn't have to become insurance experts just to get a filling fixed. We do that work so you can make decisions about your health, not your paperwork."

Common questions about dental insurance, claims, and coverage at Havens Family Dental.
We're in-network with Delta Dental and Anthem Blue Cross Blue Shield, and we accept PPO insurance as an out-of-network provider. Coverage details vary by employer and plan type, so the best way to confirm your specific benefits is to call our Blacklick office with your insurance card information before your visit.
In-network means we have a negotiated fee agreement with your insurance company, which can lower your out-of-pocket costs. Out-of-network means your plan reimburses you or us at a different rate, but PPO plans in particular often still pay a meaningful portion of your treatment. We'll give you an estimate either way before any work begins.
Yes. Our front office team submits claims directly to your insurance carrier after your appointment, whether you're covered through Delta Dental, Anthem Blue Cross Blue Shield, or a PPO plan. If your insurer requests additional information or documentation, we follow up on your behalf so the process stays off your plate.
You're still welcome at our practice. We see many patients without insurance and discuss cost-effective treatment plans and payment options during your visit. Visit our Financial Info page to learn more about how we help make care accessible without coverage.
Call our Blacklick office at 614-762-2737 with your insurance card details, and our team will look into your remaining benefits, deductible status, and annual maximum ahead of your visit. We're also glad to answer questions by phone about specific procedures and expected coverage.
Our current network focus is Delta Dental, Anthem Blue Cross Blue Shield, and PPO plans accepted out-of-network. Medicare generally does not cover routine dental care in any case. If you have a Medicaid managed dental plan, please call our office directly so we can confirm whether we're able to accommodate your specific plan.
We'll always explain your estimated out-of-pocket portion before treatment starts, based on the information your plan provides to us. For larger procedures, we can discuss payment arrangements so the remaining balance is manageable and there are no surprises on your final statement.
Simply call us at 614-762-2737 or let our front desk know when you arrive for your next appointment. We recommend updating us anytime you change employers or plans so your benefits and claims stay accurate and current.
